Bradley University requires a health form on any student taking more than six credit hours. This health form is located on the Health Services website. The first page requires basic information and does not require a physical. The 2nd page concerns immunization. Vaccination dates must be verified by a physician signature. Bradley University requires 2 MMR (measles, mumps, and rubella) vaccinations both given after your first birthday, a tetanus vaccination series within the last 10 years, a quadrivalent meningitis vaccine given after your sixteenth birthday, if you are under the age of 22 the initial series of COVID-19 vaccine and answers to the 6 tuberculosis questions. Bradley University recommends hepatitis B, HPV, and varicella vaccinations and yearly flu vaccines.
All information must be received for students to receive their grades for the current semester or register for any future classes!
Bradley University Health Services and Counseling does not bill private insurance for services provided. Each student taking more than six credit hours is assessed a Health Service fee. This fee covers all visits to Health Services and Counseling as well as some basic labwork that is listed on our website. Students are encouraged however, to keep a copy of their current health insurance card on file with Health Services.
Students should check with their insurance company prior to arriving at Bradley University to determine which health care provider in Peoria, OSF-St. Francis or Unity Point – Methodist and Proctor, they should use if needed. Additionally, labwork not covered under the health fee will be sent to the student’s health care provider of choice and they will bill their insurance. In addition, with any imaging, prompt care, or emergency room visits it is important for the student to know which health care system is covered by their insurance. Students should also have a copy of their insurance to pick up prescriptions at any of the local pharmacies.
Bradley University Health Services and Counseling does not provide written excuses for class. If a student is seen in Health Services for an illness, the student may sign a release of information and the professor may call or e-mail Health Services for verification. If a student is expected to miss several days of class, Health Services will notify Student Support Services, who will in turn notify all of the student’s professors. This includes but is not limited to diagnoses such as influenza, mononucleosis, pneumonia, concussions, and students recommended to go home for medical reasons.
Students with chronic medical conditions that may occasionally cause them to miss class should schedule an appointment with the Medical Director to discuss their case. They can then notify Health Services if they are having a flare up of their chronic medical condition. If the student is noted to have frequent flares, they will be brought in to discuss management of their condition. Students that are scheduled to have surgery or other medical procedures done can bring documentation to Health Services and we will assist with notification.
